Apply for Funding
Grant requests are considered from nonprofit, charitable organizations that are tax exempt under Section 501 (c) (3) of the IRS Code. To be considered for a grant, your organization should submit the following information:
A proposal limited to 4 pages describing the proposed project. The proposal should include the following information:
- Name and address of the organization
- Contact name, title and phone number
- Geographic area and people served
- Need for the project
- Goals and Objectives
- Activities to be undertaken
- Changes or improvements that will result
- Line item budget identifying projected expenses and income
- Funding sources committed and those approached
- Other nonprofit or public partners for the project/work
- Evaluation plans or how the results will be measured
Attachments
- Copy of organization letter of tax exempt status from the IRS
- Most recent financial statement
- List of board of directors of the organization
